Notes: Obesity is the metabolic disease with major rate
of prevalence in western countries1. The WHO
declared in 1997 that obesity was an epidemic
and represented a threat for health. In fact,
obesity is considered to be the second reason
of avoidable death (after smoking) in the world.
In general, surgery is indicated in people with
morbid or supermorbid obesity unable to lower
weight with diet and exercise. It is known that
pharmacological and psychological treatments
are effective only in short-term. In patients with
body mass index of 40 or more this efficacy is
lower, being bariatric surgery the most effective
method in those cases. The main contraindications
for this surgery are alcohol or other
substance abuse, limited intellectual capacity
or serious mental disorders. In patients with
morbid obesity who search treatment, there is
an increased prevalence (40%-60%) of psychiatric
morbidity (depression, anxiety disorder,
eating disorder, personality disorder, substance
dependence). With this case report we discuss
psycopathology of morbid obesity, psychiatric
evaluation and pre-surgery treatment.
La obesidad es la enfermedad metabólica con
mayor tasa de prevalencia en el mundo occidental1.
En 1997 la OMS declaró que la obesidad
se había convertido en una epidemia mundial
y representaba una amenaza para la salud. De
hecho, la obesidad se considera la segunda causa
de muerte evitable (tras el tabaquismo) en el
mundo. En general la cirugía está indicada en
obesidad mórbida o supermórbida que no bajan
peso con dieta más ejercicio. Es conocido que
los tratamientos farmacológicos y psicológicos
son eficaces sólo a corto plazo. Esta eficacia es
menor con índice de masa corporal superior a 40,
siendo la cirugía bariátrica el método más eficaz.
Entre las principales contraindicaciones de cirugía
bariátrica, se incluyen: el abuso de alcohol o
sustancias, alteraciones en la capacidad intelectual
o trastornos mentales graves. En pacientes con
obesidad mórbida que buscan tratamiento, hay
un incremento en la prevalencia (40%-60%) de
morbilidad psiquiátrica (depresión, trastorno de
ansiedad, trastorno de la conducta alimenticia,
trastorno de personalidad, fármacodependencia)
