<dc:description>El abordaje del consumo de tabaco es responsabilidad de todo profesional sanitario que recibe a un paciente fumador en su consulta, debido a que dicho consumo supone un factor de riesgo importante (y evitable) para su salud. La principal herramienta de los profesionales sanitarios para el abordaje inicial del tabaquismo es la intervención breve, en la cual se analiza la predisposición para dejar de fumar, se aconseja al paciente y se le ofrece la ayuda adecuada a sus deseos. Las intervenciones más eficaces son aquellas que son más intensivas y que combinación de intervenciones psicológicas con terapia farmacológica. El problema del consumo de tabaco tiene mayor repercusión en las personas con trastornos psiquiátricos, pues está demostrado que existe una mayor prevalencia de tabaquismo en este grupo en comparación con la población general. Además, estas personas suelen fumar mayor número de cigarrillos, presentar mayor dependencia de la nicotina y peores tasas de cesación que la población general. Una combinación de factores biológicos, psicológicos y sociales podría explicar esta mayor prevalencia de consumo de tabaco entre las personas con patología mental, aunque existen variaciones entre los diversos trastornos. Los pacientes con patología psiquiátrica tienden a dejar menos su consumo cuando lo intentan. Diversos factores son responsables de la eficacia de los tratamientos, pero no se ha sido capaz aún de encontrar una causa que lo explique fehacientemente. Para conseguir mejores tasas de cesación en personas con trastornos psiquiátricos se deben intensificar las intervenciones. El factor clave de esta intensificación es procurar dedicar más tiempo a estas personas y emplearlo en profundizar en los componentes adictivos de la conducta. No es estrictamente necesario recurrir con estos pacientes a técnicas distintas a las empleadas en la población general. En función del trastorno concreto que se padezca, el abordaje y tratamiento de la cesación tabáquica puede presentar diferentes características. En ocasiones, al igual que ocurre con otros pacientes, es necesaria la derivación a unidades especializadas.</dc:description>
<dc:description>The therapeutic approach to tobacco use is a responsibility of every health professional who receives a smoker patient in his praxis. This is due to the fact that this consumption poses a great -and avoidable- risk for the health of the patient. The first and main tool that we, health professionals, have in our approach to a smoking patient are brief interventions, where the predisposition of the patient to quit smoking is analyzed, advise is provided to the patient and, according to his/her expectations, help is offered. The interventions which are most effective are those which combine intensive psychological support with drug therapy. Cigarette smoking has a greater impact on people with psychiatric disorders. Compared with general population, they have is a higher prevalence of smoking. They also tend to smoke more cigarettes, have higher nicotine dependence and worse cessation rates than the general population. This increased vulnerability to smoking is probable best explained by a combination of biological, psychological and social factors. Cessation rates in psychiatric patients tend to be lower than in general population. Different factor may be responsible for this lower effectiveness of treatments in psychiatric patients, but no cause for explaining this has been properly found.For better cessation rates to be attained in people with psychiatric disorders, interventions should be intensified. The key factor of this intensification is to employ more time with the patients in order to be able to help/her to deepen in the addictive components of his/her. It is not strictly necessary to use with these patients any psychological or pharmacological tools different from those employed in general population. Depending on the characteristic of the psychiatric disorder, the management of smoking cessation may require different intervention abilities. Sometimes, as it happens with other patients, referral to a specialized unit is absolutely necessary.</dc:description>
